Sports Physicals
$45 including SCAT Sport Concussion Assessment Tool

Our Providers use a standardized method of evaluating concussion at baseline.  A copy of this tool is attached to school physical form and a copy is provided for parents of athlete.  If your child is playing a contact or projectile sport where concussion is possible, a sports physical with SCAT, is a must.

DOT physicals
$120 (includes vision, urine hearing testing)

Well-women exams
$100, plus pathology ($52) and wet mount ($15)

If you are uninsured or we are not contracted with your insurance plan, we charge $100 and $52 charge to us for sending the pap to a pathologist. Physicians Assistants Rachel and Shantel are available if you prefer a female provider for your annual exams.

Environ skin care line
$31-$285
Environ is a skin care line that can only be prescribed by a physician and uses the ingredient in Retin-A.  The key difference between this product and other systems using Retin-A is that the “A” stands for ACID.  Environ uses the “ESTER” form of Retinoid  which is much more gentle.  It also has histologic evidence that since this product penetrates deeper into the skin, it thickens the skin, thereby reducing wrinkles without injections.  Check out their website for more details.

  For your convenience, we offer a wide array of blood tests that save you time and money.  Each test uses blood from your finger tip and returns a result in approximately 5 minutes.  This allows for quicker diagnoses and treatment rather than the traditional method of taking a lab slip and reporting at another facility for lab draw.

  Dr Glick will usually provide osteopathic manipulation at the time of visit in an effort to use multiple approaches to promote healing.

  with chiropractors, physical therapists, naturopathic providers, acupuncturist, counselors as well as coordinating patient care with all of the patients specialists.

Who and What We Treat

  There isn’t one type of patient that Health Quest Family Medicine isn’t willing to see. 
  
  
• Child, individual, couples, and group counseling
  While Dr Glick doesn’t have a counseling license or special training, he believes that immediate short therapy sessions with resolution strategies can be quite effective.  Health Quest Family Medicine has a network of counselors and therapists that he can refer when appropriate.  Our providers believe that most patients have a stigma about seeing a counselor or therapist and usually there is a monetary barrier.  If you are an established patient and present with a stress or grieving processes, who could do a better job faster? 
  
  
• Nursing or group home care
  Health Quest Family Medicine understands the barriers to care of home bound patients by managed care doctors.  These patients are either poorly managed over the phone or when there is a concern, they are carted off to the hospital and seen by a new physician who likely won’t get the whole story and just button the patient up temporarily and sent home.  The advantage of having our Providers present to the patients bedside is to communicate with the patients, carry out the patient, the family and caregivers wishes and coordinate care so the patient can live more comfortably and more dignified, in most cases.  Our Providers are also available to the family of the patient to discuss the care and answer questions or concerns.
